What is a Firearm Owner Identification?

The Firearm Owner Identification card is designed to identify those individuals that are eligible to acquire and possess firearms and firearm ammunition. It was an initiative established in 1968 by the Firearm OWNER’S Identification Act as a part of the public safety initiative in Illinois.

This means that to legally possess ammunition or firearms in the States of Illinois, you must have a Firearm Owners Identification card. This is issued through the Illinois State Police and can be obtained by any qualified individual.

Information Contained on the Firearm Owner Identification Form

The Firearm Owner Identification (FOID) form is a two-page form that collects information about the applicant for the FOID card. Below are the details that are contained in the FOID form:

Personal Information: This part requires that applicants fill out their last name and first name, mailing address in Illinois, city, county, and any previous name of the applicant. They also have to identify whether they are male or female and also their race.

Questionnaire Section: The next section of the Firearm Owner Identification form is a questionnaire containing several questions that the applicants must answer. It requires that you fill out your place of birth and whether you are a citizen of the United States.

Some questions you have to answer include whether or not you have been convicted of a felony and if you have been a patient at a mental institution within the last five years before your application. It also asks whether or not you are addicted to narcotics and intellectually disabled. These are some of the questions you have to answer in this section.

Applicants under 21 Details: If you are under 21 years and you want to fill out a Firearm Owner Application form, you must provide details about your guardians. You have to provide the name of your parent or guardian, your date of birth, and your Illinois Driver’s License or State Identification card. You also have to sign as well as your parents or guardians.

You are required to attach your passport photograph to the form after filling it out and submit it to the nearest police station in the State of Illinois.

Who Needs Firearm Owner Identification Form?

Any resident of the State of Illinois that possesses or acquires firearms, firearm ammunition, stun guns, or tasers in the State must acquire a valid Firearm Owner Identification card. To obtain this card, you have to complete the form and submit it to the appropriate office. It should be noted that some people are exempted from obtaining this card by statute.
